CREDIT APPLICATION

For the purpose of securing credit accommodations from Automated Fastening Systems, Inc. the undersigned represents and warrants that the information given is true, and that it is given with the intent that it shall be relied upon in making a decision as to the advisability of extending credit to the undersigned. The undersigned agrees to pay all reasonable attorney fees incurred in the collection of any account resulting from the extension of credit to the undersigned if it becomes necessary to employ the services of an attorney to enforce collection. The undersigned also represents that there are no unpaid judgments, or open accounts more than ninety days past due, outstanding against the undersigned. The undersigned further acknowledges and hereby assumes liabilities or a service charge of 1 ½% per month to be added to the balance past due for a period of 30 days. The undersigned further warrants and represents unto Automated Fastening Systems, Inc. that the individuals executing this credit application are authorized by the applicant to do so and to bind the applicant to the liabilities and obligations hereinbefore set forth. The undersigned further authorizes release of account information for the purpose of securing credit with Automated Fastening Systems.

PERSONAL GUARANTEE

The undersigned endorsers, sureties, guarantors of accommodation parties (jointly and severally if more than one), in addition to all warranties and obligations imposed by unqualified endorsement, hereby (1) unconditionally, personally guarantee(s) to Automated Fastening Systems, Inc. the prompt payment in full of this Account as and when the same shall become due and payable whether by acceleration, extension of otherwise, together with all costs and attorney’s fees incurred in the collection or enforcement hereof, (2) assent(s) to and agree(s) to be bound by all the terms and conditions of this Account, including but not limited to the terms regarding confession of judgment against the undersigned, (3) consent(s) that the maturity of this Account may be extended from time to time and for any term or terms, to any indulgence granted to any of the undersigned, or to the release herefrom of any of the undersigned, all without notice to, or further consent from, or without affecting or releasing the liability hereunder of, any of the undersigned, and (4) waive(s) demand, presentment for payment, protest and notice of protest of non-payment, and any requirement that any action be taken against the maker or markers of this Account or any Collateral before enforcing guarantee.
